I can't give advice on how to get info about wipes quickly, that is to say, how to find out why you wiped, right after the wipe without doing some quick digging.
Personally, I don't use an addon to track cds of people. When they are needed on the fly, they get a callout. If its part of the mechanics, the order of damage mitigations/etc are known before we pull. If a bres is called, the order of who should be using their bres is known.
Hand of sacrifices, divine sacs, etc, pain suppression, are assigned to people. When RL calls out for dmg mitigation, the 1st person on the list uses theirs. When they call out for another, the second person uses theirs. This list is usually put together by the healers, we figure out who's cd would be best, etc, and the RL is part of that, but its something the healers work out.
Having an already known list for kicks/interrupts,bres, or dmg mitigations is your best bet. Or https://www.ssegold.com/wotlk-classic-gold. If the raiders are too casual to be be able to make their own decisions or judgement calls, you will have to. Our druids have it worked out who is gonna bres first, who is gonna bres second. We have innervates assigned, if a healer calls for one, so the healer doesnt get two and one is wasted.
Another idea is to have them confirm via comms that they used the cooldown/etc when you asked for it.
As far as knowing what happened/what went wrong, you can tell who used what ability with details, or there are probably better addons to track others' cooldowns, I use details.
With that, I can check to see cooldown usage, and who did and didn't use it. I can see how much dmg the raid took. looking at the dmg ppl took and why can be important.1st dmg taken is tank at 100k, 2nd dmg is OT at 90k, 3rd dmg is dps with 60k, 4th dps is 30k, etc. Just looking at dmg taken, you can see that the 3rd DPS having taken as much dmg as they did, a lot more than other DPS, probably screwed up. In sarth, that would be the firewall, so you can see they got hit by that, even if you didn't notice.
On thorim, our tanks were taking ridic amounts of dmg, so I looked at the breakdown. They were getting hit for 50k crits, because they weren't taunting off after unwavering strike or w/e it's called hit them. They had -200 defense rating. By looking at the dmg the tank took, I, as the healer, was able to identify what they did wrong. I was able to figure that out before we got back to the boss after wiping.
